Chicago Fire Department program brings non-emergency home visits to those with chronic health conditions

by BRIAN NADIG
A group of about a dozen “community paramedics” are part of a relatively new program in the Chicago Fire Department aimed at reducing 911 emergency calls from residents with chronic health conditions.
“We want to make sure you live a longer, healthier life,” paramedic Kyle Kaveney said at the May 27 meeting of the Jefferson Park Neighborhood Association. He runs the department’s Mobile Integrated Health division, which started about six years ago in certain communities and is now citywide.
The program is available to those adults who suffer from a chronic condition and could use help filling a “gap” in their health care, Kaveney said. The program’s home visits are offered at no charge.
It’s intended for those residents who have difficulty managing their chronic health conditions and frequently call 911. Asthma, chronic obstructive pulmonary disease, heart failure, hypertension and diabetes are among the chronic conditions which the program focuses on.
Three out of four participants experience reduced hospital visits and enhanced ability to manage their condition independently, according to the fire department.
During the scheduled home visits, paramedics monitor vital signs, review current medications and treatments, and provide health education and other resources.
For more information on the program, call 312-489-1601 or send an e-mail message to cfdmih@cityofchicago.org.
At the association’s June 23 meeting, Kristen Cagadas discussed a new golf simulator studio, the Chicago Golf Lounge, which she and her husband opened at 5217 W. Lawrence Ave.
“If you’re by yourself, you can finish 18 holes of golf in about 45 minutes,” Cagadas said.
There are hundreds of golf courses which can be played virtually at the studio. “(Have a friend) in California? You can play together even if you’re in two different states,” Cagadas said.
The indoor studio caters to serious and recreational golfers, and even young family members can get interested in golf via the fun virtual kid activities in which children can learn to swing a golf club, according to Cagadas.
“My husband and I are passionate about golf, as well as our two children.”
